What if I need blood work?
As a Forme member, you receive up to 80% discounts off all laboratory testing. You will be billed directly from the lab at our member’s discounted rate.
Do I get a discount on my prescription drugs?
Yes, our community partner, Sunshine Pharmacy offers all our members up to 70% discounts on medications when you present your membership card.
What if I need hospitalization?
The medical membership does not cover hospitalization, since this is not health insurance. But our clinical team will pay close attention to the prevention of illness to help our members avoid hospitalization.
What if I need surgery?
Although the membership does not cover surgery, our team at Forme will advocate on your behalf and negotiate the best rate available for the specific surgery needed.
