Physical Exam for Immigration

All persons seeking legal status in the United States are required by law to present evidence from a physical examination performed by a civilian physician endorsed by the USCIS.

At the Formé Medical Center and Urgent Care, we offer a comprehensive service that, in addition to certifying and stamping your Form I-693, reviews your vaccination card and evaluates the status of preventable diseases such as mumps, measles, rubella, polio, tetanus, diphtheria and cough. whooping among others.

The basic physical exam for immigration purposes includes:

  • Eye, ear, nose, and throat exam
  • Extremity exam
  • Exam of the heart, lungs, abdomen, and lymph nodes
  • Examination of the skin and, in some cases, a chest x-ray and a blood test are requested.

Requirements

To proceed with this process, you must comply with the following:

  • Photo ID.
  • Vaccine card. If you do not have this, you must start the cycle whose cost is not in the initial rate.
  • Clinic History
  • Chest X-ray or blood test, if requested
  • Mental health record, if it exists
  • Form I-693- If you have not completed it, request one at the medical center.

The set of medical tests that are performed for this type of exam is complex. So, in order to optimize the use of your time, we recommend that you request an appointment before approaching our facilities.

If you have any questions, contact us by phone and ask for instructions. Remember that, although Formé is endorsed by the USCIS, the test results are inconclusive and may require additional testing.

The Immigration Physical Exam is not a complete medical exam and its sole intent is to search for certain medical conditions relevant to U.S. immigration law and indicated by the U.S. Public Health Service.

If you suffer or have suffered from any of the following conditions, report it to the doctor in charge of the diagnosis.

  • Tuberculosis
  • Syphilis
  • Psychiatric or mental illness
  • Alcohol or drug abuse
  • History of violent behavior
